The Real Cost Isn’t the One on Your P&L
Every claims workflow I’ve ever walked into has the same tension: speed versus accuracy. Push too fast and you pay on claims you shouldn’t. Move too slow and your dealer starts calling your competitors. Most organizations try to manage that tension with headcount and rules: more adjusters, more policy thresholds, more manual review queues.
It works until it doesn’t. The volume grows. The complexity grows. And the team that was just barely keeping up starts drowning.
What doesn’t show up on the P&L is the cost of a 4-day cycle time on a straightforward repair authorization. The dealer who stops recommending your contract because they can’t get an answer. The adjuster who burns out reviewing invoices that a well-configured model could handle in seconds. These are the costs that compound quietly until they become a crisis.

Here’s where I’ve seen the clearest, most defensible ROI:
Intake and triage
Roughly 30 to 40 percent of manual data entry in a typical claim’s operation is capturing information that already exists somewhere; in a phone call, an email, a repair estimate sitting in someone’s inbox. Conversational AI and document ingestion agents don’t replace the claim; they complete it. First-time completeness goes up. The average handle time goes down. The adjuster starts the day with a file that’s ready to work, instead of spending the first 20 minutes hunting for a VIN and a loss code.
Adjudication assistance
This is where the conversation usually gets uncomfortable, because people assume “AI in adjudication” means removing human judgment. It doesn’t. It means giving adjusters better information, faster. Coverage eligibility surfaced in real time. Historical failure rates for the component being claimed. A flagged comparison between the submitted invoice and median pricing for that repair in that region. The adjuster still makes the call, but they’re making it with context that used to take 20 minutes to assemble, now delivered in under 30 seconds.
Low-complexity, low-amount claims that fall clearly within coverage parameters; partial autonomy makes sense. Not full automation, tiered autonomy, clear thresholds, and human escalation for anything that doesn’t fit the pattern cleanly. Done right, this alone can reduce workload on routine claims to 50 percent.
Fraud and anomaly detection
The patterns that indicate fraud in a claim portfolio are rarely dramatic. They’re subtle, a repair shop that consistently bills slightly above median, a specific combination of parts that appears more often than failure data would predict, a dealer whose cycle times look clean, but whose approval rate is 15 points higher than comparable shops. Humans aren’t good at finding those patterns across tens of thousands of records. Models are. And when the model flags something, it should put it in front of a human investigator with context, not just a score.
Why Most AI Initiatives Stall (And What to Do Instead)
The graveyard of failed AI projects in this industry has a few things in common. First, they started with technology instead of workflow. Someone got excited about a model and went looking for a problem to apply it to. That’s backwards. Start with the workflow. Find the friction. Find the problem. Then ask what kind of AI: generative, conversational, or agentic, actually fits the shape of that problem.
Second, they treated it as a one-time deployment instead of a system that learns. A claims adjudication model that isn’t being retrained on adjuster outcomes isn’t getting better; it’s drifting. The feedback loop between human decisions and model behavior isn’t optional. It’s the whole point.
Third, and most importantly: they didn’t build trust. Adjusters who don’t trust a model’s suggestions will work around it. And they’ll be right to do so if the model can’t explain its reasoning. What the Operating Model Actually Looks Like
The end state isn’t a fully automated claims operation. That’s not the goal, and frankly it’s not realistic in a domain with this much regulatory complexity and edge-case volume. The goal is a tiered operating model where AI handles the predictable, and humans handle the judgment intensively.
1 Tier one: straight-through processing
Claims that fall clearly within coverage, match historical failure patterns, and have invoices within median pricing bands. These get approved automatically, with a full audit trail, and the adjuster sees them only in their completed queue.
2 Tier two: assisted adjudication
Claims that need a human but where the AI has already assembled the relevant context, flagged the anomalies, and drafted a decision rationale. The adjuster reviews, modifies if needed, and approves. Time per claim drops significantly.
3 Tier three: escalation and exception handling
Anything that doesn’t fit a clean pattern; high-value claims, fraud indicators, coverage disputes, complaints. These go to your most experienced people, with full context surfaced and a clear record of what the model saw and why it escalated.
Workforce routing ties all of this together. The right claim to the right handler at the right time, based on predicted complexity, adjuster skills and current capacity, SLA risk, and dealer tier. Not a queue, a scorecard, recalculated continuously.
The Data Question Nobody Asks Early Enough
You cannot build a predictive claims operation without historical data that’s clean, structured, and accessible. I know that’s not a revelation. But the number of organizations that get six months into an AI initiative before discovering that their historical outcomes data is siloed, inconsistently coded, or missing entirely; that number is higher than anyone wants to admit.
Before you invest in models, invest in your data foundation. Set clear definitions, standardize loss codes, and connect outcomes back to intake data. It’s not flashy, but it’s what makes AI actually work.
Where to Start
